1. What information should I bring with me to the doctor visit?
  • Medical insurance card(s)
  • Driver License or government identification
  • List of current medications
  • Medical records of previous surgeries (if applicable)
  • Patient Forms (available in our website)
  • Any radiology test that have been taken for your condition. It is your responsibility to bring these to your first appointment so that you can be examined and the films reviewed at the same time.
  • Exact co-pay amount
  • If your insurance requires a referral or authorization from your primary care physician to see a specialist, then it is your responsibility to obtain that referral before you come to your office visit. You should contact your insurance company directly to verify whether or not you require a referral.


2. What should I expect during first visit?

During your initial visit, the doctor will explain the specific test or procedure you may need, help you visualize results, and go over the risks. He may do a physical examination, ask questions about your condition, and explain appropriate treatment.


3. What are the office hours?

The practice is open Monday through Thursday from 8am to 5pm and Fridays from 8am to 12pm.


4. What insurances is Dr. Martinez participating with?

For your convenience we accept most insurance carriers. However, since there are many divisions of insurance companies, it would be in your best interest to call your insurance provider to see if Dr. Henry E. Martinez is an in-network provider.
